Helping people living with more than one long-term condition through better care, treatment and self-management.

Millions of people in the UK live with two or more long-term health conditions, known as multiple long-term conditions (MLTC). Managing multiple conditions is complex for patients and challenging for health services. This theme develops and tests approaches that help people live better and receive more joined-up care.

We work with partners to co-produce research that can be implemented nationally and adapted locally, reflecting priorities identified with patients, carers, healthcare professionals and national policy bodies.

We aim to:

  • Tackle the most pressing challenges in preventing and managing MLTC
  • Empower patients to manage their conditions through tailored interventions, including digital tools, lifestyle support and medication optimisation
  • Ensure our research reflects the diversity of people affected and benefits those experiencing the greatest health inequalities.
