Personalising activity for multiple long-term conditions

Steps to Better Health turned complex activity tracker data into simple daily movement plans for people with multiple long-term conditions (MLTC).

Through validating accurate step counts from wrist-worn devices, creating simple conversion charts from research metrics, and analysing UK Biobank data, the project proved more daily steps significantly reduce risks of early death, heart disease, diabetes, and multimorbidity.

People with MLTC often spend excessive time sitting, experience poor sleep and movement patterns, and struggle with generic exercise advice that doesn't suit their needs. Previously, complicated tracker data made it difficult for healthcare professionals to provide clear, personalised guidance or motivate patients effectively.

This led to Steps4Health, a free website built around five essential daily habits: sleeping well, sitting less, stepping regularly, raising your heart rate, and building strength.

Co-designed with patients, the Leicester Biomedical Research Centre, and the NIHR-funded M3 Study, the personalised plans adapt to all abilities with seated exercises, sleep advice, smart reminders, motivating challenges, and rewards.

Originally piloted in Leicestershire weight management programmes, Steps4Health saw 95% of offered patients sign up. They increased their daily steps by a clinically significant average of 974 steps. The tool has now gone national with over 1,000 registered users (November 2025). A mobile app is under development to make these life-changing tools accessible nationwide. 

As it scales, the app will support habit-building after interventions to sustain behaviour change and reduce complications, easing NHS pressure through prevention, self-management, and improved physical and mental wellbeing.
