Learning points from scaling up of innovations for multiple long-term conditions in Primary Care

As the prevalence of patients with multiple long-term conditions (MLTCs) in primary care rises – due to ageing populations and advancements in medical care – the need for effective and scalable interventions is critical.

Research insights 

This project evaluates the large-scale implementation of four interventions funded by the NIHR ARC MLTC theme (2021-2024) to optimise care for MLTC patients across two to seven ARC regions in England.

Methodology

Two rounds of interviews were conducted with project teams and implementation leads, totalling 19 interviews, along with an analysis of project documents. Engagement with key stakeholders helped validate findings, culminating in a workshop with six stakeholders.

Key findings

Key challenges identified include:

  • Diversity in patient needs: Addressing the varied needs of patients and healthcare providers.
  • Operational challenges: Managing demand, structural changes and workforce instability.
  • Engagement and inclusivity: Ensuring sustained involvement and inclusivity.
